The NHS Health Check programme (HC) is a 5-yearly contract with health care for adults aged 40-74 without a diagnosis of heart disease, kidney disease, stroke, or type 2 diabetes to manage Cardiovascular Disease Risk Factors (CVDRFs) for those conditions. A Health Check consists of an assessment of an individual's risks of developing a set of diseases in the future. This requires collecting a range of health information such as height, weight, smoking history, physical activity, blood pressure, and arranging for a cholesterol blood test to be performed. This is a statutory service legislated in 2012. In Stoke-on-Trent, the programme was suspended over the pandemic and continues to recover following pandemic disruption but is unlikely to achieve the national target of health check delivery to 75% of the eligible population. In Stoke-on-Trent, the programme is delivered exclusively by general practice as a universal offer. We are looking to expand the delivery of the programme and are therefore, seeking Providers who can deliver NHS Health Checks in non-healthcare settings, working with communities and using a targeted approach to reach areas with a low coverage, high health needs, o
