About this contract

Stoke-on-Trent City Council is seeking to appoint one Provider for Lot 1 and one Provider for Lot 2 to deliver two specialist short-term home-based support services for adults aged 18 and over. The services are designed to support people to remain safely within their own homes during periods of increased need, crisis, recovery, assessment or transition, whilst avoiding unnecessary hospital admissions and long-term residential care placements. Providers may bid for one or both Lots; however, each Lot will be awarded to a single Provider. Lot 1 - Night Support Service Lot 1 is a short-term overnight support service delivered through block rota arrangements. The service is intended for individuals who require short-term support, supervision, observation or reassurance during night-time hours from 11pm - 6am. The successful Provider will deliver overnight support within an individual's home, typically during periods of crisis, hospital discharge, deterioration in health, carer breakdown, increased risk or whilst assessments are being undertaken.
