The London Borough of Hammersmith & Fulham (the Council) is inviting suitably qualified and experienced providers to tender for the delivery of Community Reablement & Home Care Services. This procurement supports a continuum of support available to residents to regain or retain their independence living in the community. The services will provide support for adults in Hammersmith & Fulham by proactively taking all opportunities to build on people's strengths, maximise their independence and connect them with their community. This procurement represents a strategic redesign of the previous service model. It reflects learning from previous delivery, stakeholder engagement, and evolving adults social care priorities. The new model will have a focus on reablement enabling support to residents to regain the skills, confidence and social networks to return to their previous levels of independence as far as possible and reduce the likelihood of admission/ re-admission to hospital, or the need for long term care and support.
