Herefordshire Council is looking to commission suitably qualified providers to deliver care and support services to people in their own home. The service will enable people to meet their outcomes through the delivery of high-quality personal care and support. Providers will deliver enabling home care that meets the person's needs, maintains and increases their independence and minimises their need for formal care. The service will be provided to adults aged 18 years and over who live in Herefordshire and have been assessed as eligible for social care services. Subject to formal governance, the service is proposed to be divided into 8 Lots based on geographical location: Lot 1 - Mortimer, Lot 2 - Leominster, Lot 3 - Bromyard, Lot 4 - Ledbury, Lot 5 - Ross-on-Wye, Lot 6 - Kington, Weobley and Golden Valley, Lot 7 - Hereford North, Lot 8 - Hereford South. Bidders will be permitted to submit a tender for more than one Lot, depending on the service area they are able to cover and their capability and capacity to meet the requirements of the relevant Lot(s).
