About this contract

It delivers two core functions: 24/7 residential rehabilitation at Wellbridge House, offering psychosocial interventions, medication management, physical health support and daily living skills development; and community rehabilitation, supporting individuals in their own homes or supported accommodation through structured rehab plans, proactive case management, crisis planning, safeguarding and multi‑agency coordination
